    I authorise Axus Group and their contracted third-party companies to relocate the above equipment.
    Pricing is based on the information provided by the customer. Additional charges will be added if finisher or stairs were not disclosed on original request. I understand a $300 (+GST) charge will apply if the scheduled relocation is cancelled or changed by the customer within 24 hours of the scheduled pickup/delivery time, or if the scheduled relocation is unable to be completed at the scheduled pickup/delivery time due to access to the location of the equipment (pick up or delivery location) not being provided by the customer, insufficient or incorrect access details being provided by the customer or any other act or omission of the customer which results in Axus Group (in its sole discretion) cancelling the scheduled relocation.
